CIVIL UNION CERTIFICATES

WHO MAY OBTAIN A COPY OF A CIVIL UNION CERTIFICATE? 

  • Anyone.  Civil union certificates are open to the public.

NOTE:  Any information regarding family history which is normally found on the back of a certificate will not be reproduced for the general public. 

WHAT IS REQUIRED BEFORE YOU CAN OBTAIN A COPY OF A CIVIL UNION CERTIFICATE?

  • A Government issued picture ID (Drivers License, State ID Card) if requesting a Certified Certificate. If your request is by mail, send a copy of your ID along with the request.

  • A fee of $10.00 for each certified copy requested, each additional certified copy of the same record at the same time is $7.00. Make checks payable to Adams County Clerk.

HOW CAN YOU OBTAIN A COPY OF A CIVIL UNION CERTIFICATE?

 

You may obtain a copy of a Civil Union Certificate either in person or by requesting it by mail.  Stop by our office at 507 Vermont Street, Monday thru Friday between 8:30 a.m. and 4:30 p.m. to obtain a copy of a certificate.  Mail your request to our office. 

Download the Civil Union Certificate Request Form and mail the completed form to this office.  Either way you choose, you will need to provide us with the following information: 

  • The name of partner A (legal name used at the date of the ceremony)

  • The name of the partner B (legal name used at the date of the ceremony)

  • The date and city of the ceremony
