Who may obtain a copy of a death certificate?

A person or his/her authorized agent having a genealogical, personal, or proper interest in the record (a relative or a person who can prove a financial interest).

What is required before you can obtain a copy of a death certificate?

  • A government issued picture ID (Drivers License, State ID Card).  If your request is by mail, send a copy of you ID.

  • A fee of $12.00 for the first copy, and $9.00 for each additional copy of the same record issued at the same time.

 How can you obtain a copy of a death certificate?

        You may obtain a certified copy of a Death Certificate either in person or be requesting it by mail.  Stop by our office at 507 Vermont Street Monday thru Friday between 8:30 a.m. and 4:30 p.m. to obtain a certified copy of a Death Certificate.   Mail your request to our office.  Click here (Death Certificate Request) to download and print our application form.  Either way you choose,  you will need to provide us with the following information:
  • Decedent’s name
  • Decedent’s date of death
  • Decedent’s city of death

RECORDS OF DEATH IN ADAMS COUNTY WHICH ARE KEPT ON FILE AT THE CLERK’S OFFICE BEGIN IN THE YEAR 1878.   THE ADAMS COUNTY CLERK’S OFFICE ONLY HAS DEATH CERTIFICATES FOR THOSE PEOPLE WHO DIED IN ADAMS COUNTY.
