How to Obtain Vital Records
The Adams County Clerk’s Office serves as the official record keeper for births, deaths, and marriages that take place in Adams County. You have three options.
1. Visit our office at 507 Vermont Street, Quincy, Illinois. Our hours our Monday thru Friday, 8:30 a.m. to 4:30 p.m. Check Holiday Schedule.
2. By mail, download the appropriate form in the list below.
When mailing the form, you must include:
A. a self-addressed, stamped envelope
B. a check or money order to cover the applicable processing fees
C. a copy of the requestor’s valid identification.
