Military
Discharge
Information
on Military Discharge is located in the Illinois Compiled Statutes,
Chapter 55, ILCS, 5/3-5015, Certificates of Discharge or Release
from Active Duty.
Military
discharge information is no longer subject to public inspection.
Anyone
requesting a certified copy of a DD214 or any other military
discharge papers will be required to appear in person with proper
identification or send us the proper notarized request form.
Please send your request with a self addressed stamped envelope and
$1.00 for each certified copy requested to: Adams County Recorder,
507 Vermont St., Quincy, IL 62301.

DD214-1 is for the Veteran requesting the discharge
information.
Form
DD214-2 is for a dependent requesting the discharge
information.
